Licensed Vocational Nurse (LVN)

Reports To: Chief Clinical Officer (CCO)

Schedule: 12-hour shifts | Days, Nights, or Weekends

About Nexus Neurorecovery Center – Conroe

Situated on a peaceful, wooded 26-acre campus, Nexus Neurorecovery Center – Conroe offers the Rise Program, a comprehensive neurological rehabilitation program for adults recovering from severe traumatic brain injury (TBI), stroke, spinal cord injury, and other neurological diagnoses.

This campus is also home to the Life Care Program, which provides long-term residential care for individuals who have sustained serious, life-altering injuries and require ongoing clinical and behavioral support.

Position Summary

The Licensed Vocational Nurse (LVN) provides direct nursing care and rehabilitation support for adults recovering from neurological injuries or living in long-term care. Reporting to the Chief Clinical Officer (CCO), the LVN assists with medication administration, documentation, and clinical interventions under the supervision of a Registered Nurse (RN).

The LVN collaborates with the interdisciplinary team to ensure compassionate, patient-centered care that fosters recovery and stability in a structured, therapeutic environment.

Key Responsibilities

  • Provide hands-on nursing care and assistance with daily activities for adult patients.
  • Administer prescribed medications and document accurately within EHR systems.
  • Support therapeutic interventions and assist in executing individualized care plans.
  • Monitor patients’ physical and behavioral status, reporting significant changes promptly to supervising RNs.
  • Use approved de-escalation and crisis intervention techniques to ensure patient and staff safety.
  • Participate in interdisciplinary team meetings to enhance care coordination and recovery outcomes.
  • Promote patient independence, dignity, and adherence to safety and rehabilitation goals.
  • Uphold compliance with all facility policies, procedures, and clinical standards.

LICENSURE & CERTIFICATIONS

  • Current Texas LVN license – required
  • CPR / BLS Certification – required
  • De-escalation Training – required within 30 days of hire
  • Experience in neurorehabilitation, long-term care, or behavioral health preferred
